What does this PR do?

Adds graphify-opencode to the ecosystem's Plugins table, linking to its published npm package. The plugin connects Graphify's hosted MCP for code intelligence and repository memory, with native OAuth and a /graphify command.

How did you verify your code works?

• Checked the added table row and ran git diff --check.
• Confirmed the public source repository and npm package.
• Installed published version 1.0.0 in a clean OpenCode 1.18.35 profile; MCP registration, /graphify registration, and live OAuth discovery passed. Full account authorization was not part of this check.

The dev branch (v1) only takes critical fixes, so I'm closing this docs change there. The v2 branch has the same packages/web/src/content/docs/ecosystem.mdx plugin table, so you're welcome to open this entry as a pull request against v2 instead. When you do, please check that it works with OpenCode v2 (@opencode/cli): v2 is a separate implementation, and your testing was on 1.18.35.
