fix: migrate scoped memory metadata

Migrates Codex scoped memory writes and reads from reserved sm_scope to searchable agent_scope.

Changes

• Write agent_scope for automatic capture and explicit personal/project saves.
• Filter canonical search and profile requests by agent_scope, including custom canonical tags.
• Keep legacy compatibility containers intentionally unfiltered.
• Update status/docs and document the backend-first release gate.

Rollout dependency

Do not release this plugin change until https://github.com/supermemoryai/mono/pull/2895 is deployed and its metadata/vector migration has completed.

Testing

• npm run typecheck — passed.
• npm test — 71 passed.
• npm run build — passed.
• git diff --check — passed.
