Reviewer's GuideThe PR restores a one-time stream error listener in the body setter for failures that occur before piping or when a response is never piped, then marks streams at pipeline handoff so each error is reported exactly once; regression tests cover delayed, HEAD, and listener-registration behavior. 🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win
Synchronize both tests with the error event.
If the filesystem error occurs after 50 ms, the first test can pass through pipeline() without testing the new listener. The HEAD test can check errors before the error arrives and fail intermittently. Wait for the relevant stream or application error event instead of using a fixed delay.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
Track piping for the current body stream.
If a callback replaces the body after an earlier stream starts piping, _bodyPiped remains true. An error from the replacement stream then reaches this listener but never reaches ctx.onerror, even though the replacement stream was never piped. Reset the flag when the body changes, or track which stream was handed to pipeline().

Description
Since #1893, the
bodysetter no longer attaches anerrorlistener to stream bodies and relies onStream.pipeline()inrespond()instead. That only covers errors emitted afterrespond()pipes the stream. If the stream fails before that, nothing is listening, Node throws, and the process goes down. This happens when:await next()followed by async work);ctx.respond = false. The docs still say that
.onerroris added as a listener when a stream is set as the body, and that is how Koa 2 behaved. This change adds back a one-time
errorlistener in the setter that forwards toctx.onerror. It only forwards while the stream is still the body andrespond()has not piped it yet. Oncerespond()hands the stream topipeline(), it marks the response, and from then on the pipeline callback is the only place that reports errors, so nothing gets reported twice. Replaced streams (the koa-compress wrapping case from #1913) and bodies cleared by an empty status work as before. The
body.test.jslistener test goes back to its original form ("should add error handler to the stream, but only once"). Two new tests inrespond.test.jscover the await and HEAD cases; both crash the test run without the fix.Checklist
