Implements the schema-20/protocol-12-compatible v2.5.4 recovery plan: candidate-first persistence, typed evidence obligations, bounded enrichment and retry fencing, truthful pipeline counters and semantic-collapse classification, generalized Guidance V5.1 with route/worker-consumption receipts, actionable repair states, and exact-SHA release/promotion gates.
