Was this intentionally?

Does that mean we give essentially up on OpenMP? I would not mind, as that makes life much more complicated, but I feel like this change sneaked in without being discussed.

Thanks @dschwoerer ! Not entirely intentional, but not very worried about this: Most field ops should now be going through BinaryExpr template expressions, so the choice of execution method (serial/OpenMP/CUDA etc.) is in one place: The BinaryExpr::evaluate function https://github.com/boutproject/BOUT-dev/blob/next/include/bout/fieldops.hxx#L405

Eventually I think generated_fieldops and the index_derivs machinery will be deleted and replaced with template expressions. It'll slow compilation but I think it's the only way we can hope to get good performance on GPUs.
