Generate the starting dashboard during setup so a fresh clone never 404s

A fresh clone had no index.html or outputs/, so the first `npm run serve`
showed "Not found" at /. Setup now runs plan:list-dashboard at the end,
and the zero-plans overview tells new users how to create their first
plan. Verified end-to-end with a clean clone: setup, serve, HTTP 200,
empty state with the create-first-plan hint.
